CLIENT RIGHTS
v Client/Caregiver has the right to be treated with dignity and respect
v Client/Caregiver has the right to considerate and respectful therapy, in a safe environment, free from all forms of abuse or harassment. The client may exercise these rights without regard to sex or cultural, economic, educational, or religious background or the source of payment for care
v Client/Caregiver has the right to full privacy concerning their ABA treatment program which includes case discussion, consultation, examination, and treatment are confidential and should be conducted discretely.
v Client/Caregiver has the right to confidential treatment of all communications and records pertaining to his or her care and his or her visit at the clinic. Except when required by law, clients are given the opportunity to approve or refuse the release of information.
v Client/Caregiver has the right to the delivery of therapeutic services in a culturally competent manner
v Client/Caregiver has the right to file a complaint or grievance by filing out a “Client Complaint Form” provided by LDC. Please see the Client Complaints Procedure for additional information
v Client has the right to remain free from seclusion or restraints of any form that are not clinically necessary or are used as a means of coercion, discipline, convenience, or retaliation by staff
v Client/Caregiver has the right to know about laws that relate to their rights and responsibilities
v Client/Caregiver has the right to know their rights and responsibilities in the treatment process and to make recommendations regarding the organization’s right and responsibilities policy
CLIENT RESPONSIBILITIES
Ø Client/Caregiver has the responsibility to provide the most current insurance coverage as well as notify their provider (Louisiana Developmental Center) about changes to coverage which can include change of carrier as well as loss of insurance
Ø Client/Caregiver has the responsibility to provide accurate and complete information concerning his/her present complaints, past or current therapies, and other matters relating to his/her therapeutic treatment
Ø Client/Caregiver has the responsibility to treat those giving them care with dignity and respect
Ø Client/Caregiver has the responsibility to follow the treatment plan of care
Ø Client/Caregiver has the responsibility to notify their provider (Louisiana Developmental Center) about any medication changes
Ø Client/Caregiver has the responsibility to attend therapy as scheduled (outlined in the Louisiana Developmental Center Informed Consent for Treatment document) or notify their provider as soon as possible regarding canceled sessions
Ø Client/Caregiver has the responsibility to report perceived risks in care and unexpected changes in condition to his/her provider (Louisiana Developmental Center)
Ø Client/Caregiver has the responsibility to follow the policies and procedures of Louisiana Developmental Center
Ø Client/Caregiver has the responsibility to be considerate of the rights of other Client/Caregivers as well as clinic personnel
Ø Client/Caregiver has the responsibility to be respectful of his or her personal property and that of other persons in the clinic.
Ø Client/Caregiver has the responsibility to report abuse and fraud
Ø Client/Caregiver has the responsibility to report any changes to their contact information or the information of people approved to pick the client up from the provider (Louisiana Developmental Center)
